Description
Ohio’s Hospice is hiring a full-time Triage Nurse to provide clinical support and triage for hospice patients and families through its Call Center. The role assesses incoming calls, prioritizes needs, coordinates clinical services, communicates with the interdisciplinary team, documents care in the electronic medical record, and supports continuity of care. The position is on-site at the Dayton/Centerville location, operates from 6:00 AM to 6:30 PM with every-other-weekend rotation, and requires an Ohio RN license, CPR certification, a valid driver’s license, IV and vascular access skills, and CHPN certification or certification within two years of employment.
