Aligned Modern Health is hiring Nurse Practitioners for its functional medicine practice. The role involves conducting patient assessments and functional diagnostic testing, developing personalized treatment plans, prescribing nutraceuticals and medications, coordinating care, counseling patients, maintaining electronic health records, and staying current with functional, integrative, and lifestyle medicine. Full-time employment is preferred, with part-time options considered on an individual basis; an active Illinois APRN license is required to start, and providers based in Florida, Michigan, Tennessee, or Texas are preferred. The position offers a starting base salary guarantee of $130,000, bonus and revenue-share opportunities, and medical, dental, and vision benefits.
