Description

Reach for Resources Inc. is seeking a full-time Waiver Case Manager-DH (Developmental Disabilities) to provide case management services to adults needing waiver and state plan services, along with medical, social, educational, and other necessary services. This role involves coordinating supportive services to address individuals' basic, social, recreational, health, educational, and vocational needs, managing a caseload of no more than 50 individuals. The case manager will also be responsible for documenting services, providing community-based services, and engaging in public relations and networking.

Additional Information

Required Previous Experiences: One year of experience in education or treatment of people with developmental disabilities or related conditions, or a minimum of one course that specifically focuses on developmental disabilities.
At least one year of case management experience
at least two years of experience working with individuals with disabilities
Preferred Qualifications: Skilled in identifying and assessing recipient needs
Knowledgeable about local community resources and how to use them
Ability to function independently, interact with, and relate to various individuals
Encourages change and provides a positive and supportive environment
Effectiveness in decision-making
Ability to handle crisis situations and adhere to supervisor's directives
