Description
The Wmo consultant will conduct home visits for residents in Rijswijk, assess their needs, identify suitable care solutions, collaborate with colleagues, and document findings in a support plan and the Zorgned registration system. The role requires completed Wmo training or HBO-level working ability, current Wmo knowledge and experience, and knowledge of home adaptations or psychiatric and somatic conditions. It is a 32–36-hour-per-week position with a gross monthly salary of up to €5,096, a one-year contract with an intention of permanent employment, and flexibility to work where needed.
