Youth Advocate Programs, Inc. is hiring a part-time Wraparound Facilitator to partner with youth experiencing serious emotional and behavioral challenges and their families. The role provides intensive case management, care coordination, Wraparound Team facilitation, resource connection, advocacy, documentation, and services in homes and community settings. A bachelor's degree in a relevant field, strong communication and organizational skills, Microsoft 365 proficiency, reliable transportation, a valid driver's license, and current auto insurance are required; EHR experience and bilingual Spanish/English ability are preferred.
