Wiltshire Council is hiring a Reablement Support Worker to provide home-based support to individuals after hospital discharge, helping them achieve personal goals and work with occupational therapists to develop reablement plans. The role requires rota availability between 7am and 10pm, 365 days a year, travel for customer visits, a valid UK driving licence, and access to a vehicle unless an alternative arrangement is agreed. It pays £13.92–£14.36 per hour plus 10% unsocial-hours plussage for 27 hours per week and offers NVQ apprenticeships and upskilling opportunities.
