Description
The Case Manager assesses participant needs, develops and coordinates individualized service treatment plans, provides mobile community-based support, advocates for participants, builds community networks, and documents compliance with agency, funder, and licensing standards. The role reports to the Program Director, is part-time, and requires a bachelor’s degree in Human Services or a related field, at least two years of direct support experience in behavioral health, addiction recovery, or IDD services, a valid driver’s license, and intermediate computer skills. The position is primarily field-based within the program’s service delivery area and requires driving participants to appointments.
