St. Joseph Center is hiring a full-time, non-exempt E6 MDT Case Manager to work with a multidisciplinary homeless street outreach team. The role provides street-based outreach, navigation, supportive services, housing-first and harm-reduction advocacy, client referrals, transportation assistance, documentation in HMIS, and coordination with community providers. The position requires a bachelor's degree in social work, human services, or a related field, experience with vulnerable populations, knowledge of homeless services and related interventions, and the ability to operate a motor vehicle.
