Description

UTMB Health's Correctional Managed Care program is hiring a Clinical Practice Pharmacist to provide advanced medication therapy management and direct or consultative patient care for underserved populations served by the Texas Department of Criminal Justice and Texas Juvenile Justice Department. The pharmacist will oversee specialty medications, conduct medication reconciliation and chart reviews, manage drug therapy through telemedicine, answer drug information consultations, support formulary and clinical guideline development, monitor high-cost medication use, collaborate with multidisciplinary care teams, participate in quality and medication-safety initiatives, educate and precept pharmacy learners, and support regulatory compliance. The position is split approximately 50% between the Conroe and Huntsville offices, with possible work in correctional or juvenile detention environments. A Bachelor of Science in Pharmacy or Doctor of Pharmacy, an ASHP-accredited PGY1 residency or three years of related experience, and Texas pharmacist licensure or eligibility are required; security clearance and pre-employment drug testing are also required.
