Advantmed is hiring a full-time Nurse Practitioner to provide in-home wellness and risk-adjustment assessments for diverse patient populations across Oregon. The role includes health evaluations, chronic-condition management, preventive screenings, patient education, care coordination, and documentation for value-based care. Providers must travel within a 55-mile radius of their assigned home location, with statewide travel scheduled at least two weeks in advance.
