St. Joseph Place is hiring a Service Coordinator to support older adults living in its HUD 202 Senior Housing community. The role assesses residents' social, health, and support needs; provides case management, referrals, and service coordination; connects residents with transportation, meals, counseling, legal, personal assistance, and healthcare resources; develops individualized service plans; advocates for residents; and coordinates community programs and volunteer opportunities. Applicants need a bachelor's degree in social work or a related field or at least five years of social services experience, with one year of experience working with older adults or in a geriatric setting preferred.
